How to File A Claim

Filing a claim is easy. Follow these simple steps.

1. Bring the Happy Tails Claim Form during your visit to the Vet clinic. You can download the form here.
2. Request the treating Vet to complete the “Medical” section of the form. The fee for obtaining the medical report shall be borne by you.
3. Send a soft copy of the completed form together with the necessary supporting documents (indicated in the claim form) to [email protected].
   Income may contact you for any additional documents or information required to assess your claim.
4. If you are filing a claim for the first time, please include the following documents:
   1. Pet’s clinical medical examination report for insurance enrolment
   2. Records of all required vaccinations
   3. Latest medical records, if any
